Abstract (English): This article presents a clinical case in which a conventional frenectomy was performed for prosthetic and aesthetic purposes on a 60-year-old female, presented who came to the clinic for diastema in the midline of the upper incisors and a papillary and penetrating labial frenum. This anatomical condition interfered with the proper fit the previously dental veneers, affecting both the aesthetics and functionality of the restorative treatment. After a detailed periodontal evaluation to ensure the health of the gingival tissues, a frenectomy was performed using a rhomboidal excision technique with a conventional scalpel. The procedure enabled precise removal of the frenum, promoting healing and preparing the site for proper rehabilitation. The surgical outcome was successful, as it reduced the abnormal of the frenum and facilitated the placement of new dental veneers with proper adaptation. This significantly improved the patient's smile aesthetics and oclusal function. The conventional technique was chosen over laser technique due to its effectiveness, accessibility, and predictability, especially relevant in resource-limited settings. The literature reviewed supports this choice, as it indicates that both techniques yield comparable long-term clinical outcomes, though traditional methods may provide more efficient short-term healing. This case highlights the importance of assessing the labial frenum's anat omy in restorative treatments. Neglecting this factor can result in poor retention, adaptation, or esthetic outcomes. A multidisciplinary approach-incorporating periodontal, surgical, and prosthetic considerations-was essential for treatment success. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): Se presenta un caso clínico en el que se realizó una frenectomía convencional con fines protésicos y estéticos, de una paciente de 60 años que acudió a consulta por un diastema en la línea media de los incisivos superiores y un frenillo labial de inserción papilar y penetrante. Esta condición dificultaba la correcta adaptación de las carillas dentales previas, afectando tanto a la estética como a la funcionalidad del tratamiento. Después de la evaluación periodontal detallada para garantizar la salud de los tejidos gingivales, se llevó a cabo una frenectomía con técnica de exéresis romboidal mediante un bisturí convencional. El procedimiento permitió eliminar el frenillo de manera precisa, y favorecer la cicatrización, así como la preparación del sitio para una rehabilitación adecuada. El resultado quirúrgico fue exitoso, ya que se logró reducir la tracción anómala del frenillo y facilitó la colocación de nuevas carillas dentales con buena adaptación, lo que mejoró significativamente la estética de la sonrisa y la función oclusal de la paciente. Se optó por la técnica convencional en lugar del láser, debido a su efectividad, accesibilidad y predictibilidad, factores relevantes en contextos con recursos limitados. La literatura revisada respalda esta elección, ya que señala que los resultados clínicos de ambas técnicas son comparables a largo plazo. Este caso resalta la importancia de evaluar la anatomía del frenillo en los tratamientos restauradores.
