| Abstract (English): | Introduction: the overweight and obesity in pediatrics has been increasing in recent decades. Objective: to analyze the adherence to international guidelines for the actions taken in the diagnosis and management of childhood overweight/obesity by the medical staff of in Primary Care Units (UMF) of the Mexican Social Security Institute (IMSS). Material and methods: this was an observational, cross-sectional, and retrospective study. The study was conducted at UMF No. 28 in Mexico City. Records of patients aged 6 to 16 years with overweight or obesity, treated during 2018 and 2019, were identified. Data were collected on whether the correct nutritional status diagnosis was made, the actions taken to identify other comorbidities, and the actions taken to manage overweight/obesity. The statistical analysis was descriptive. The protocol was approved by the UMF's Research and Ethics Committee. Results: 223 patients with overweight (44%) and obesity (56%) were studied. Family physicians made the correct nutritional diagnosis in 68 patients (30.5%). Regarding the screening for comorbidities, blood pressure measurements were taken in 83 (37.2%) patients, but these were not interpreted in any case. Hyperglycemia was investigated in 25.1%, dyslipidemia in 23.3%, and elevated transaminases in 4%. As for management, the recommendation to engage in physical activity was 28.2%, and to reduce the consumption of sugary drinks was 14.3%. Conclusions: in pediatric patients with overweight or obesity seen at the primary care level, actions for the detection of overweight/obesity, its comorbidities, and its management are not routinely performed. | Abstract (Spanish): | Introducción: el problema del sobrepeso y obesidad en la edad pediátrica ha venido aumentado en las últimas décadas. Objetivo: analizar el cumplimiento de las guías internacionales de las acciones realizadas para el diagnóstico y manejo del sobrepeso/obesidad infantil, por parte del personal médico de una Unidad de Medicina Familiar (UMF) del Instituto Mexicano del Seguro Social. Material y métodos: estudio observacional, transversal y retrospectivo. El estudio se llevó a cabo en la UMF No. 28 de la Ciudad de México. Se identificaron expedientes de pacientes de seis a 16 años, con sobrepeso u obesidad, atendidos durante los años 2018 y 2019. Se registró si se había efectuado el diagnóstico correcto del estado de nutrición, las acciones para la identificación de otras comorbilidades, así como las acciones para su manejo. El análisis fue de tipo descriptivo. El protocolo fue aprobado por el Comité de Investigación y Ética de la UMF. Resultados: se estudiaron 223 pacientes con sobrepeso (44%) y obesidad (56%). Los médicos familiares establecieron el diagnóstico nutricional correcto en 68 pacientes (30.5%). Sobre la búsqueda de comorbilidades, en 83 (37.2%) se registró la presión arterial, pero en ningún caso se interpretó. Se buscó hiperglucemia en el 25.1%, dislipidemia en el 23.3% y transaminasemia en el 4%. En cuanto al manejo, la recomendación para hacer actividad física fue en el 28.2%, y para disminuir el consumo de bebidas azucaradas en el 14.3%. Conclusiones: en pacientes pediátricos con sobrepeso/obesidad atendidos en el primer nivel de atención, al parecer no se realizan de forma rutinaria las acciones para la detección de sobrepeso/obesidad, de sus comorbilidades y para su manejo.
