| Abstract (English): | Objective: To describe our experience with VRAM flaps in complex thoracic and perineal reconstructions. Materials and Methods: Multicenter case series of five patients undergoing complex reconstruction: two involving the chest wall and three involving the perineal region. Surgical indication, technique employed, early postoperative course, and complications were recorded. Long-term satisfaction was assessed through a patient survey. Results: No signs of flap compromise were observed. In the recipient site, three infections, two wound dehiscence, and two seromas occurred; at the donor site, one incisional hernia was reported. The mean operative time was 4 hours, and the average hospital stay was 21 days. All flaps maintained long-term viability. Patients undergoing perineal reconstruction reported persistent pain or discomfort, with a significant impact on quality of life. Discussion: In this small series, the VRAM flap behaved as a useful and versatile option for the reconstruction of extensive perineal and chest wall defects, although it was associated with relevant morbidity and the results should be interpreted with caution. | Abstract (Spanish): | Objetivo: Describir la experiencia en el uso de colgajos VRAM para reconstrucciones complejas torácicas y perineales. Materiales y Métodos: Serie de casos multicéntrica de cinco pacientes sometidos a reconstrucción compleja: dos en pared torácica y tres en región perineal. Se registró la indicación quirúrgica, técnica empleada, evolución postoperatoria temprana y complicaciones. A largo plazo se evaluó satisfacción mediante encuesta. Resultados: No se observaron signos de sufrimiento de colgajo. En el sitio receptor se presentaron tres infecciones, dos dehiscencias y dos seromas; en el sitio donante, una hernia incisional. El tiempo quirúrgico promedio fue de 4 horas y la estadía hospitalaria media de 21 días. Todos los colgajos mantuvieron su vitalidad a largo plazo. Los pacientes con reconstrucción perineal reportaron dolor o molestias persistentes, con impacto relevante en su calidad de vida. Discusión: En esta pequeña serie, el colgajo VRAM se comportó como una técnica útil y versátil para defectos extensos en región perineal y pared torácica, con alta tasa de supervivencia y aplicabilidad en escenarios complejos.
