| Abstract (English): | This report presents the case of a man in his third decade of life with a complex abdominal course, the analysis of which highlights critical issues related to diagnostic opportunity, continuity of care, and clinical follow-up. Initially, the condition was interpreted as a functional disorder; however, the persistence of symptoms and the poor response to conservative treatment prompted further evaluations. At a second unit, complementary investigations suggested an abdominal wall defect consistent with an inguinal hernia, and elective surgical management was indicated. After surgery and hospital discharge, the patient developed persistent pain, vomiting, and progressive clinical deterioration, with reassessments in which warning signs were not integrated in a timely manner. Readmission led to the identification of an abdominal complication requiring urgent intervention, during which an intestinal injury was identified. This case highlights the importance of avoiding premature diagnostic closure, strengthening post-procedural surveillance, and improving the integration of clinical information as key elements of patient safety. | Abstract (Spanish): | Se presenta el caso de un hombre de 27 años con una evolución abdominal compleja, cuyo análisis permite identificar áreas críticas en la oportunidad diagnóstica, la continuidad asistencial y el seguimiento clínico. En una fase inicial, el cuadro fue interpretado como un trastorno funcional; sin embargo, la persistencia de los síntomas y la escasa respuesta al tratamiento conservador motivaron nuevas valoraciones. En una segunda unidad, los estudios complementarios sugirieron un defecto de pared abdominal compatible con hernia inguinal, por lo que se indicó manejo quirúrgico electivo. Tras la intervención y el egreso hospitalario, el paciente presentó persistencia del dolor, vómito y deterioro progresivo, con revaloraciones en las que los signos de alarma no se integraron de manera oportuna. El reingreso permitió documentar una complicación abdominal que requirió intervención urgente, durante la cual se identificó lesión intestinal. El caso pone de relieve la importancia de evitar el cierre diagnóstico prematuro, fortalecer la vigilancia posterior a procedimientos y mejorar la integración de la información clínica como elementos clave para la seguridad del paciente.
