Abstract (English): Introduction: Diabetes is a chronic metabolic disease that presents complications such as diabetic foot syndrome, expressed by ulcerations susceptible to infection by multidrug-resistant bacteria, making more difficult its treatment with conventional antibiotics and increasing the risk of amputation. Faced with this challenge, phage therapy is regarded as an alternative treatment for diabetic foot infection control. Objectives: To collect and analyze the available evidence about phage therapy efficacy in diabetic foot infection, to present its clinical potential and guide future research. Methods: A literature review was carried out in 11 databases, compiling 16 articles that evaluated efficacy, routes of administration, adverse events and treatment effects using individual phage therapy or combined with antibiotics. Results: The studies show positive treatment results, decreasing bacterial load and without reports of adverse effects. Efficacy changes depending on the complexity and presence of polymicrobial infections. Conclusions: The analysis of vitro and animal studies, as well as case reports, indicate positive results with phage therapy in multi-resistant diabetic foot infections, thereby finding a potential alternative to more invasive procedures such as amputation. However, large-scale controlled and randomized clinical trials are needed to obtain accurate data on its efficacy and safety. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): Introducción: la diabetes es una enfermedad metabólica crónica que genera complicaciones como el síndrome de pie diabético, que se manifiesta mediante ulceraciones propensas a infectarse por bacterias multirresistentes, lo que dificulta el tratamiento con antibióticos convencionales y aumenta el riesgo de amputación. Ante este desafío, la fagoterapia se considera como una alternativa terapéutica para el control de infecciones en el pie diabético. Objetivos: recopilar y analizar la evidencia disponible sobre la eficacia de la fagoterapia en infecciones del pie diabético para exponer su potencial clínico y orientar futuras investigaciones. Métodos: se realizó una revisión bibliográfica en 11 bases de datos y se seleccionaron 16 artículos que evaluaron la eficacia, vías de administración, eventos adversos y efectos del tratamiento utilizando fagoterapia individual o combinada con antibióticos. Resultados: los estudios demostraron los resultados positivos del tratamiento, con disminución de la carga bacteriana y sin efectos adversos reportados. La eficacia varía según la complejidad y la presencia de infecciones polimicrobianas. Conclusiones: los estudios in vitro, en animales y los reportes de casos analizados, evidencian resultados positivos con la fagoterapia en infecciones multirresistentes del pie diabético, encontrando una alternativa potencial a los procedimientos más invasivos como la amputación. Sin embargo, es necesario realizar ensayos clínicos controlados y aleatorizados a gran escala para obtener datos precisos sobre su eficacia y seguridad. [ABSTRACT FROM AUTHOR]
