| Abstract (English): | The identification and differential diagnosis of non-epileptic paroxysmal events is of great importance, as it avoids expensive, unnecessary, and potentially dangerous studies and treatment for the patients. The differential diagnosis of epilepsy is numerous and varies depending on the patient's age (childhood, adolescence, or adulthood). It includes sleep disorders such as somnambulism and night terrors, cardiovascular disorders (syncope) extrapyramidal abnormal movements including restless legs, dyskinesias, chorea, and tremors, sleep disorders in addition to paroxysmal events that comprises the functional seizures, which represent 20% of referrals to epilepsy units. The primary care physicians can initiate the differential diagnosis process by taking an adequate medical history and performing basic studies and the referring to a neurologist who, through specialized neurodiagnostic studies, can confirm or eliminate the diagnosis of epileptic seizures. | Abstract (Spanish): | La identificación y diagnóstico diferencial de los eventos paroxísticos no epilépticos es de gran importancia y evita la realización de estudios y tratamientos costosos, innecesarios y potencialmente peligrosos para el paciente. El diagnóstico diferencial de la epilepsia es múltiple y varía dependiendo de la edad del paciente (infancia, adolescencia o vida adulta); incluye trastornos del sueño (sonambulismo, terrores nocturnos), trastornos cardiovasculares como el síncope, movimientos anormales extrapiramidales incluyendo piernas inquietas, discinesias, corea y temblores; trastornos del sueño, además de eventos paroxísticos que incluyen crisis funcionales las cuales representan el 20% de las referencias a unidades de epilepsia. El médico de primer contacto puede iniciar el proceso diagnóstico diferencial efectuando adecuada historia clínica, estudios básicos y posteriormente apoyarse en un médico neurólogo, quien mediante estudios de gabinete especiales podrá establecer o descartar el diagnóstico de crisis epilépticas.
