Abstract (English): Introduction: Breast cancer is the leading cause of cancer-related death in women worldwide and in Chile. Early screening and comprehensive patient characterization are essential to optimize diagnosis and treatment. Our objective was to clinically, demographically, and histopathologically characterize patients treated at the breast surgery outpatient clinic of the Hospital San Juan de Dios from 2023 to 2024. Materials and Methods: 2118 electronic medical records corresponding to admissions to the breast surgery outpatient clinic from 2023 to 2024 were reviewed. Results: Of 2118 patients, 1951 had data in their medical records. 99% were women (mean age: 52 years); 630 cases were malignant (33%), with infiltrating ductal carcinoma predominating (69%). The malignancy rate progressively increased with BI-RADS category (6% in BI-RADS 3; 53% in BI-RADS 4; 95% in BI-RADS 5; p < 0.001). Diagnostic yield improved when combining mammography and ultrasound (66.9%) vs. using a single modality (mammography 49.2%, ultrasound 28.3%). In women over 50 years of age, the yield reached 72.3%. 14 percent presented with metastasis, and 41.5% received neoadjuvant treatment. Discussion: The combination of mammography and ultrasound significantly improves the diagnostic yield in breast cancer, especially in women over 50 years of age. The BI-RADS system was shown to be a robust predictor of malignancy. Conclusion: The high proportion of advanced cases underscores the need to strengthen early detection strategies and optimize diagnostic pathways in the public health system. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): Introducción: El cáncer de mama es la principal causa de muerte oncológica en mujeres a nivel mundial y en Chile. El screening precoz y una caracterización integral de las pacientes son fundamentales para optimizar el diagnóstico y tratamiento. Nuestro objetivo es caracterizar clínica, demográfica e histopatológicamente a los pacientes atendidos en policlínico de mama del servicio de cirugía del Hospital San Juan de Dios 2023-2024. Material y Método: Se revisaron 2.118 fichas electrónicas correspondientes a los ingresos hechos al policlínico de cirugía de mama 2023-2024. Resultados: de 2.118 pacientes, 1.951 tenían datos en ficha clínica. 99% fueron mujeres -media: 52 años-; 630 casos fueron malignos (33%), predominando el carcinoma ductal infiltrante (69%). La tasa de malignidad aumentó progresivamente con la categoría BI-RADS (6% en BI-RADS 3; 53% en BI-RADS 4; 95% en BI-RADS 5; p < 0,001). El rendimiento diagnóstico mejoró al combinar mamografía y ecografía (66,9%) vs utilizar una sola modalidad (mamografía 49,2%, ecografía 28,3%). En mayores de 50 años, el rendimiento alcanzó 72,3%. Un 14% debutó con metástasis, y el 41,5% recibió tratamiento neoadyuvante. Discusión: La combinación de mamografía y ecografía mejora significativamente el rendimiento diagnóstico en cáncer de mama, especialmente en mujeres mayores de 50 años. El sistema BI-RADS demostró ser un predictor sólido de malignidad. Conclusión: La alta proporción de casos avanzados subraya la necesidad de fortalecer estrategias de detección precoz y optimizar los circuitos diagnósticos en el sistema público de salud.
