| Abstract (English): | Introduction: The evaluation and management of early rectal tumors (superficial parietal invasion) represent a diagnostic and therapeutic challenge. Over the last decade, classical transanal resection has been displaced by minimally invasive techniques such as TAMIS, which allow access to more proximal rectal tumors and enable en bloc specimen excision with a higher rate of negative margins. The main indication for TAMIS is extraperitoneal rectal tumors, particularly when early cancer limited to the submucosa (pT1) is suspected. Objective: To evaluate the outcomes of the TAMIS technique in patients with extraperitoneal rectal tumors. Materials and Methods: A retrospective cross-sectional study based on the review of medical records of patients who underwent TAMIS for rectal tumors between January 2022 and December 2024. Results: Nineteen patients were included, with a mean age of 63 years, presenting rectal tumors located at a mean distance of 6.6 cm from the anal verge and a mean tumor size of 2.5 cm. The mean operative time was 37 minutes. Ninety percent of patients were discharged within the first 24 hours. Two Clavien–Dindo grade I complications were recorded. Ten benign lesions and nine malignant lesions were resected, achieving complete en bloc resection with negative margins in all cases. At a mean follow-up of 26 months, 95% of patients remain without evidence of local or distant recurrence. Discussion: TAMIS may represent a safe and potentially curative alternative for benign rectal tumors and early pT1 rectal cancer, reducing the morbidity associated with radical resections. | Abstract (Spanish): | Introducción: El estudio y tratamiento de los tumores rectales tempranos (invasión parietal superficial) es un desafío diagnóstico y terapéutico. En la última década la resección transanal clásica, ha sido desplazada por técnicas mínimamente invasivas como el TAMIS que permiten abordar tumores rectales más proximales y extirpar la pieza en bloque con mayor tasa de márgenes negativos. La principal indicación de TAMIS son los tumores de recto extraperitoneal, sobre todo ante la sospecha de cáncer temprano limitado a la submucosa (pT1). Objetivo: Evaluar los resultados de la técnica TAMIS en pacientes con tumores de recto extraperitoneal. Material y Métodos: Estudio transversal retrospectivo basado en la revisión de fichas clínicas de pacientes sometidos a TAMIS con tumores de recto entre enero de 2022 y diciembre 2024. Resultados: Se incluyeron 19 pacientes, edad promedio 63 años con tumores rectales ubicados a una distancia media de 6,6 cm del margen anal y tamaño promedio de 2,5 cm. El tiempo operatorio medio fue de 37 minutos. El 90% de los pacientes fue dado de alta dentro de las primeras 24 horas. Se registraron dos complicaciones Clavien-Dindo grado I. Se resecaron 10 lesiones benignas y 9 malignas, logrando resección completa en bloque con margen negativo en todos los casos. En el seguimiento promedio a 26 meses, el 95% de los pacientes permanece sin evidencia de recurrencia local ni a distancia. Discusión: TAMIS puede ser una alternativa segura y potencialmente curativa para tumores rectales benignos y en cáncer temprano pT1, reduciendo la morbilidad asociada a las resecciones radicales.
