Abstract (English): Dental care is a multifactorial and prevalent condition, especially among pediatric patients, potentially compromising aesthetics, function, and quality of life. This study aims to report a clinical case of aesthetic and functional rehabilitation of primary anterior teeth in a pediatric patient presenting with extensive lesions caused by caries and wear due to bruxism. The treatment was performed using an adhesive restorative technique with acetate crowns filled with Bulk Fill composite resin, combined with the installation of the myofunctional appliance "Ocluso Guide". The clinical steps included crown preparation, acid etching, adhesive application, resin insertion and finishing, followed by adaptation of the occlusal device. The combination of acetate crowns and Bulk Fill resin provided proper marginal adaptation, satisfactory aesthetics, and mechanical resistance to parafunctional forces, while reducing clinical time and improving patient comfort. It is concluded that the use of this technique, associated with bruxism control through myofunctional devices, represents an effective alternative for restoring primary teeth with significant structural loss, promoting positive aesthetic, functional, and psychosocial outcomes. Abstract (Portuguese): A cárie dentária é uma condição multifatorial e prevalente, especialmente em pacientes odontopediátricos, podendo comprometer a estética, a função e a qualidade de vida. Este trabalho tem como objetivo relatar um caso clínico de reabilitação estética e funcional de dentes decíduos anteriores em paciente pediátrica com lesões extensas decorrentes de cárie e desgaste provocado pelo bruxismo. O tratamento foi conduzido por meio de técnica restauradora adesiva utilizando coroas de acetato preenchidas com resina composta Bulk Fill, associada à instalação do aparelho miofuncional "Ocluso Guide". As etapas clínicas incluíram o preparo das coroas, condicionamento ácido, aplicação de adesivo, inserção da resina e acabamento, seguidos da adaptação do dispositivo oclusal. A associação das coroas de acetato à resina Bulk Fill proporcionou adequada adaptação marginal, estética satisfatória e resistência mecânica frente às forças parafuncionais, reduzindo o tempo clínico e favorecendo o conforto da paciente. Conclui-se que a utilização dessa técnica, aliada ao controle do bruxismo por meio de dispositivos miofuncionais, constitui uma alternativa eficaz para a restauração de dentes decíduos com grande perda estrutural, promovendo resultados estéticos, funcionais e psicossociais positivos.
