Abstract (English): There are many regulations for the design-development and the manufacture of biomaterials. The Center for Governmental Control of Medical Devices is the institution that evaluates this kind of products in Cuba, to guarantee safety and efficacy in their medical use. This evaluation requires the conformity with national or international technical standards. Certification of products and quality systems are carried out by the National Office for Standardization (NOS) and requires the compliance with pertinent national standards. Both evaluation processes demand from the manufacturer to keep a quality management system, including some requirements of the ISO 9000 standards. Quality system certification involves the compliance of the manufacturer requirements to guarantee customer satisfaction and the regulatory and technical requirements. In this paper the requirements of the quality system needed for product certification and medical registration in Cuba are discussed and their relation to ISO 9001 standard. Also, the results obtained by the Center of Biomaterials of Havana University in this field are shown. Nowadays, neither product certification nor regulatory requirements for medical registration include quality system requirements in the design development process, customer focus, continuous improvement, internal quality auditing or management revision. Product certification demands the control of documents, quality assay records, including specifications, having a difference with government assessment that does not have these requirements. For the medical registration, compliance with different requirements is claimed depending on the risk classification of the medical device. More requirements are asked for the assessment of Class III medical devices. Medical registration demands the control of supplies, product traceability and validation of special processes, specially the sterilization processes. ISO 9000 quality system certification includes the compliance with quality requirements demanded by product certification and medical registration. The Center of Biomaterials has implanted a ISO 9001:2000 Quality Management System (QMS) in design- development and production that has been certified by NOS, having a solid support for medical registration of new products in the field of medical materials and for product certification. An effort has to be done by Cuban medical devices manufacturers to implant and certify their QMS and also to develop national technical standards for products assessment. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): La industria de los biomateriales, incluyendo la etapa de diseño-desarrollo, es un sector ampliamente regulado. En Cuba, el Centro de Control Estatal de Equipos Médicos realiza la evaluación de estos productos con el objetivo de garantizar la seguridad y eficacia durante su utilización en el Sistema Nacional de Salud. Esta evaluación estatal exige el cumplimiento de los requisitos técnicos contemplados en las normas nacionales o internacionales más actuales. La certificación de productos que realiza la Oficina Nacional de Normalización (ONN) sí exige la conformidad con una norma nacional. Una semejanza entre los dos procesos (la evaluación estatal y la certificación de productos) es que exigen el establecimiento de sistemas de calidad en la producción, mediante el cumplimiento de algunos de los requisitos plasmados en las normas ISO 9000. La certificación según estas normas expresa el cumplimiento de aquellos requisitos del Sistema de Gestión de la Calidad (SGC) que puede utilizar la organización para garantizar la satisfacción del cliente, lo que supone el cumplimiento de los requisitos técnicos y legales pertinentes. En este trabajo se analizan los requisitos de gestión de la calidad exigidos para la Certificación de Productos y el Registro Médico y su relación con los requisitos de la Certificación ISO 9001:2000. También se muestran los resultados obtenidos por el Centro de Biomateriales de la Universidad de la Habana en este campo. Ni la certificación de productos, ni la evaluación estatal para el Registro Médico incluyen entre sus requisitos los relacionados con la gestión de la calidad en el proceso de diseño-desarrollo, el enfoque al cliente, la mejora continua, los procesos de auditorías internas, la revisión por la dirección y otros. La certificación de productos, a diferencia de la evaluación estatal, exige el control de los documentos y los registros de inspección y ensayos, así como el control de los cambios en las especificaciones. El Registro Médico plantea diferentes exigencias sobre la base de la clasificación del producto en cuanto al riesgo, mientras mayor es este relacionado con el producto más requisitos de calidad se incluyen en la evaluación para el registro. Como base para este trabajo, se tomó la Clase III que es la que más exigencias plantea. A diferencia de la certificación de productos, el Registro Médico exige un mayor control de los suministros, así como la trazabilidad del producto y la validación de los procesos especiales, con especial énfasis en los procesos de esterilización. La aplicación de los SGC según las normas ISO 9000, permite el cumplimiento de los requisitos de calidad tanto para el Registro Médico, como para la certificación de productos. El Centro de Biomateriales ha implantado un SGC en el diseño-desarrollo y la producción, el cual ha sido certificado por la ONN, creando de esta forma una sólida base para el Registro Médico de nuevos materiales de uso médico y la certificación de productos. Los productores cubanos de equipos médicos deben realizar un esfuerzo por implantar y certificar sus SGC y también desarrollar normas técnicas nacionales para la evaluación de sus productos.
