Abstract (English): Introduction. The surgeon's task, in the treatment of patients with facial fractures, is to provide in aesthetical appearance and a functional situation, similar to the "pre-traumatic" state. The evaluation of patients with craniofacial trauma has several aspects. There are several surgical specialties involved in the treatment of patients with facial fractures. The results, as far as the effectiveness, between each one of these have not been described previously in extensive form. Material and methods. The clinical files of the patients operated with any type of facial fractures in Mexico's City, Hospital Central Militar were reviewed, in a 4 year period between 2003 and 2007. A demographic analysis and a comparative study were made between the specialties of plastic surgery and head & neck surgery (HN), as well as between plastic surgery and maxillofacial surgery. Results. Of the 108 patients with facial fractures, 88% were male and 12% female. The higher incidence was the active duty military personnel in 73% of the cases, followed by the military social security holders in 19% of the cases. Automobile accidents were the main cause of injury. In significant form the HN specialty gave treatment to a greater number of patients (58%). In smaller amount plastic surgery 19% and maxillofacial surgery with 18%. The most common diagnostic method was plain x-ray films (25%). Thirty eight percent of the patients required medical evaluation by other specialties, mainly by ophthalmology, in one-third of the cases. The mandible was the most frequent site of injury in 47% of the cases. The specialty of plastic surgery had better results than the HN specialty regarding the following variable: length of hospital stay, days of stay until the surgery, days of stay from the day of the surgery to date of hospital departure, surgical time, analgesic requirements and postoperative complications (p < 0.05). There were no statistical significant differences between plastic surgery and maxillofacial surgery (p > 0.05). Conclusions. The population more commonly affected by facial fractures was male patients, in the third decade of life and in an active duty military administrative situation. Most of the patients were treated by the HN specialty; but the statistical tendency showed that the number of patients treated by the specialty of plastic surgery is increasing with time. The specialties of plastic surgery and maxillofacial surgery offer a similar and "better" quality of attention for the patient facial fractures, superior to the one granted by the HN specialty. Abstract (Spanish): Introducción. La responsabilidad del cirujano encargado del tratamiento de pacientes con fracturas faciales es la de proporcionar una apariencia estética y una situación funcional, similares al estado "pre-traumático". Son varias las especialidades encargadas del tratamiento de los pacientes con fracturas faciales. Los resultados, en cuanto a la efectividad, entre cada una de éstas no se han descrito en forma extensiva con anterioridad. Material y métodos. Se revisaron los expedientes clínicos de los pacientes operados por fracturas faciales, en el Hospital Central Militar, en un periodo de cuatro años entre 2003 y 2007. Se realizó el análisis demográfico y un estudio comparativo entre las especialidades de cirugía plástica y otorrinolaringología (ORL), así como entre cirugía plástica y cirugía maxilofacial. Resultados. De los 108 pacientes con fracturas faciales, 88% pertenecieron al sexo masculino y 12% al sexo femenino. La máxima incidencia en cuanto al grupo de edad se encontró entre lo 21 y 30 años. La población afectada con mayor frecuencia fue la de los militares en el activo (73%), seguida de los derechohabientes en el 19% de lo casos. El principal mecanismo de lesión fueron los accidentes automovilísticos. En forma significativa la especialidad de ORL dio tratamiento a un mayor número de pacientes (58%). En menor cantidad cirugía plástica 19% y cirugía maxilofacial con 18%. El método diagnóstico más común fueron las radiografías simples (25%). Treinta y ocho por ciento de los pacientes requirieron ser valorados por otras especialidades, principalmente por la de oftalmología, en un tercio de los casos. Las fracturas de mandíbula fueron las más frecuentes en el 47% de los casos. La especialidad de cirugía plástica tuvo mejores resultados que la especialidad de ORL cuando se compararon las variables de: días de estancia intrahospitalaria total, días de estancia hasta la cirugía, días de estancia desde el día de la cirugía hasta la fecha de egreso, tiempo quirúrgico, menores requerimientos de analgésicos y menores complicaciones postoperatorias (p < 0.05). Estas diferencias en comparación con la especialidad de cirugía maxilofacial no fueron significativas (p > 0.05). Conclusiones. La población más comúnmente afectada por la fracturas faciales fueron los pacientes de sexo masculino, en la tercera década de la vida y en situación de militares en el activo. La mayor parte de los pacientes fueron tratados por la especialidad e ORL; pero la tendencia estadística mostró que el número de pacientes atendidos por la especialidad de cirugía plástica va en aumento. Las especialidades de cirugía plástica y cirugía maxilofacial brindan una calidad de atención "mejor" para el pacientes con fracturas faciales, superior a la otorgada por la especialidad de otorrinolaringología.
