Abstract (English): The existence of multiresistent bacteria in the hospital and community environment has become a public health issue worldwide. At the present time, a bacterial strain can develop multiple resistance mechanisms to almost all available antibiotics. Likewise, an antibiotic can be inactivated by different mechanisms and bacterial strains. We present the fatal case of a inmunodeficient patient that during hospitalization acquires a severe bacterial infection. The isolated strain was Acinetobacter baumannii. The antibiogram showed resistance to multiple wide spectrum antibiotics. We describe the most common resistance mechanisms with emphasis in the blaOXA-23 clone chromosomally encoded. Finally we describe general recommendations and universal methods of control and prevention of resistance. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): La existencia en el ambiente hospitalario y comunitario de gérmenes dotados de complejos y múltiples mecanismos de resistencia se ha convertido en un problema de salud pública mundial. En la actualidad, una cepa bacteriana puede desarrollar varios mecanismos de resistencia frente a uno o varios antibióticos. Del mismo modo, un antibiótico puede ser inactivado por distintos mecanismos por diferentes cepas y especies bacterianas. Este artículo presenta el caso fatal de una paciente inmunocomprometida quien durante su hospitalización adquiere una infección bacteriana grave. El aislamiento demostró la presencia del germen Acinetobacter baumannii. El antibiograma reportó una resistencia múltiple a antibióticos de amplio espectro. Se describen los mecanismos de resistencia más comunes y se hace énfasis en la presencia de un clon identificado con el gen de resistencia blaOXA-23 codificado cromosómicamente. Finalmente se hacen recomendaciones generales sobre las medidas universales de prevención y control de resistencia y las particulares para el caso que se describe. [ABSTRACT FROM AUTHOR]
