| Abstract (English): | Contemporary transformations in the ageing process, as a social event, have led to the acknowledgement of diversity, heterogeneity and ecological dimension of individual and social ageing . The environments we live in today and the way we relate with them determine how aging is faced and the meaning it acquires. Maturescence implies conquering the positive aspects of ageing on the way to achieving personal integrity. It makes manifest that it is about movement, flexibility and it encompasses women of mature age and those in the early stages of ageing. Growing old is a stage marked by loss and also gains. The longer women live, the more vulnerable and fragile they become from a functional, medical and physiological approach. This process can be delayed or speeded by the so-called modulatory factors. Because of the socio-demographical profile of aging in Argentina, of its epidemiological profile, of the characteristics of its health system, together with cultural conceptions on health/ disease/attention/ care , we consider the systematic implementation of a nursing care at home, going from case management, aimed at elderly people, their familias and whoever acts as main career. Families having elderly people within them should become --in the short term-- the centre of public and health policies, due to socio-demographical changes the population in our country is undergoing, differentiated by region. Within families women continue to be those entrusted with the care of the elderly, the chronic patients and the disabled without any assistance from health services and little social and economic support to face them, this situation involving risks for their own health. It is necessary to offer efficient, effective services based on appropriate theories and methodologies, tending to provide care, preserving the health of those in charge of care and the possibility or remaining at home for those willing to do so. Scheduled home care is more cost effective than hospital admíttance produced by decompensations or complications. This is so not only taking into account the hospital bill for admission but also the cost families assume of having to get to hospital and for the old person the possibility of being abandoned, as many times is the case. | Abstract (Spanish): | Las transformaciones contemporáneas del proceso de envejecimiento, como acontecimiento social, han llevado al reconocimiento de la diversidad, la heterogeneidad y la dimensión ecológica del envejecimiento individual y social. Los entornos en los cuales se vive y la manera como nos relacionamos con ellos determinan cómo se concibe y afronta el envejecimiento y qué significado se le atribuye. La madurescencia implica la conquista de los aspectos positivos del envejecimiento, en la consecución de su integridad personal. Pone de manifiesto que se trata de movimiento, flexibilidad y abarca a mujeres de mediana edad y vejez temprana. La vejez es una etapa signada por pérdidas y también por ganancias. La mayor longevidad de las mujeres las vuelve vulnerables y frágiles desde un enfoque funcional, médico y fisiológico. Este proceso se puede ver retrasado o adelantado por los llamados factores moduladores. El perfil socio-demográfico del envejecimiento en nuestro país, el perfil epidemiológico, las características del sistema de salud y las concepciones culturales sobre salud/enfermedad/atención/cuidado son las razones por las cuales consideramos que debería implementarse sistemáticamente el cuidado de enfermería domiciliario, desde la gestión de casos, destinado a personas de edad avanzada, a sus familias y a quien opere como cuidador principal. Las familias con ancianos deberían ser ya, o lo serán a la brevedad, centro de políticas públicas sociales y de salud, en virtud de los cambios socio-demográficos que está atravesando la población de nuestro país, con diferencias por región. En las familias siguen siendo las mujeres las depositarias de los cuidados de la mayoría de los ancianos, enfermos crónicos y discapacitados sin asistencia de servicios de salud y escaso apoyo social y económico para afrontarlos, con riesgos para su propia salud. Es necesario ofrecer servicios eficientes, eficaces, fundados en teorías y metodologías apropiadas, tendientes a brindar cuidados, preservando la salud de quienes se hacen cargo del cuidado, y la posibilidad de permanecer en el hogar para quienes así lo quieran. El costo-beneficio de la atención domiciliaria programada es mayor que cada internación producto de descompensaciones y complicaciones, en virtud no solo de gastos de internación, sino de lo que implica para las familias desplazarse y para el anciano la posibilidad de ser abandonado, como tantas veces hemos visto.
