| Abstract (English): | Identify which patients are going to bleed during a surgery or invasive procedure is desirable. The coagulation tests as the prothrombin time (PT), the activated partial thromboplastin time (aPTT), bleeding time (BT) and platelet count are routinely made with this purpose in the preoperative evaluation. But, really these laboratory tests can predict what patients are going to bleed? The answer with the current knowledge is: No. The most efficient method for prediction of bleeding is the clinical history, but still, most surgical bleeding are not by coagulopathy, they are caused by factors related to surgical technique, and therefore are not susceptible to be predicted. This article describes the recommendations for the assessment of bleeding risk prior to surgery or invasive procedures and discusses the value of preoperative coagulation tests. | Abstract (Spanish): | Es deseable identificar qué paciente va a sangrar en una cirugía o procedimiento invasivo. Con este fin, en la valoración preoperatoria son solicitadas rutinariamente las pruebas de coagulación como el tiempo de protrombina, el tiempo de tromboplastina parcial activada, el tiempo de sangrado y el conteo plaquetario. Pero, ¿realmente estos exámenes de laboratorio predicen quién sangrará? A la luz de los conocimientos actuales, la respuesta es no. Las mejores herramientas para predecir el riesgo de sangrar en un procedimiento quirúrgico son el interrogatorio y la exploración física, sin embargo, la mayoría de los sangrados no se debe a coagulopatía sino a factores relacionados con la técnica quirúrgica y, por lo tanto, no son susceptibles de predicción. En este artículo se describen las recomendaciones para la evaluación del riesgo de sangrado previo a cirugías o a procedimientos invasivos y se discute el valor de los exámenes preoperatorios de la hemostasia.
