Abstract (English): The increase bone turnover is the most important fact at the physiopathology of postmenopausal osteoporosis. At the molecular level it is the increase of the RANK-L the principal mediator who allows the osteoclasts formation, survival and development of them. Multiple models have been in use in preclinical investigations for evaluating the effects of the inhibition of the RANKL, between which it is included on OPG’s over-expression in mice and transgenic mouse and by means of other models that include the treatment with recombinant OPG. The results of these studies show that RANKL’s inhibition improves the DMO as well as the geometry and resistance of the bone. Several references clinical trials have investigated the potential of denosumab as antiresorptive drug for the prevention and treatment of bone disease. It is a monoclonal antibody, the first fully human, which were recently published results of randomized clinical trials to eight years for increasing BMD in different locations and five years for the prevention of vertebral and hip fractures. Abstract (Spanish): El incremento del remodelado óseo, luego del déficit estrogénico, es uno de los mecanismos fundamentales de la osteoporosis postmenopáusica. Desde el punto de vista molecular es el incremento del RANK-L, el principal mediador que permite la formación, supervivencia y desarrollo de los osteoclastos. Para evaluar los efectos de la inhibición del RANK-L se han utilizado múltiples modelos, entre ellos la sobreexpresión de osteoprotegerina en ratones y ratas transgénicas y mediante otros modelos, que incluyen el tratamiento con osteoprotegerina recombinante. Los resultados de estos estudios muestran que la inhibición del RANK-L incrementa la densidad mineral ósea y mejora la geometría y resistencia del hueso. En diversos ensayos clínicos se ha investigado el potencial de denosumab como fármaco antirresortivo para tratar y prevenir las osteopatías. El denosumab es un anticuerpo monoclonal, totalmente humano, del que hace poco se publicaron los resultados de ensayos clínicos aleatorizados a ocho años en donde se demuestra el incremento de la densidad mineral ósea en diferentes localizaciones y a cinco años para prevenir fracturas vertebrales y de cadera. Se revisan los datos fundamentales del mecanismo de inhibición del RANK-L y los resultados de los diversos ensayos clínicos con denosumab. [ABSTRACT FROM AUTHOR]
Abstract (French): L’augmentation du remodelage osseux, puis du déficit oestrogènes, est l’un des mécanismes fondamentaux de l’ostéoporose post-ménopausique. Depuis le point de vue moléculaire est l’augmentation du Rank-L, le médiateur principal qui permet la formation, de survie et développement des ostéoclastes. Pour évaluer les effets de l’inhibition de RANKL plusieurs modèles ont été utilisés, y compris la surexpression ostéoprotégérine chez des souris transgéniques et des rats par d’autres modèles, qui comprennent le traitement par l’ostéoprotégérine recombinant. Les résultats de ces études montrent que l’inhibition de la densité de RANK-L augmente minérale osseuse et de la géométrie et améliore la solidité des os. Dans plusieurs essais cliniques ont étudié le potentiel de denosumab en tant que médicament inhibiteur de la résorption pour traiter et prévenir les maladies osseuses. Le denosumab est un anticorps monoclonal entièrement humain, qui a récemment publié les résultats d’essais randomisés à huit années au cours desquelles la preuve de l’augmentation de la densité minérale osseuse à différents endroits et cinq ans pour prévenir les fractures vertébrales et de la hanche. Nous passons en revue les éléments fondamentaux du mécanisme d’inhibition de RANK-L et les résultats de divers essais cliniques observés avec dénosumab. [ABSTRACT FROM AUTHOR]
Abstract (Portuguese): Remodelação óssea aumentada após a deficiência de estrogénio, é um dos mecanismos fundamentais da osteoporose pós-menopáusica. De um ponto de vista molecular é aumentado RANK-L, o que permite a formação de mediador principal e sobrevivência do desenvolvimento dos osteoclastos. Para avaliar os efeitos da inibição do RANK-L vários modelos foram utilizados, incluindo a superexpressão osteoprotegerina em ratos transgénicos e os ratos por outros modelos, que incluem o tratamento com osteoprotegerina recombinante. Os resultados destes estudos mostram que a inibição do sistema RANK-L aumenta a densidade mineral óssea e geometria e melhora a resistência do osso. Em vários estudos clínicos investigaram o potencial de denosumab como droga anti-reabsorção para o tratamento e prevenção de doenças dos ossos. Denosumab é um anticorpo monoclonal totalmente humano, que recentemente publicou os resultados de ensaios clínicos randomizados a oito anos em que a evidência do aumento na densidade mineral óssea em locais diferentes e de cinco anos para prevenir fraturas vertebrais e de quadril.
