| Abstract (English): | This case study describes the conceptualization and the cognitive-behavioral treatment planning within the model of the Acceptance and Commitment Therapy (ACT) for a 21-year-old patient affected by depression and anxiety, with a family story made by parents neglect. The treatment proposed the use of the ACT Matrix (Polk, Schoendorff, Webster, & Olaz 2016), as an instrument for the conceptualization and the therapy to help the patient in increasing the psychological flexibility, and live a life toward her values. At the same time we start an initial training of Mindfulness (Preziosi, 2016; Kabat-Zinn, Segal, Teasdale, & Williams, 2010) to increase the willingness of the patient in accepting the physical sensations and the disturbing thoughts. Since the patient felt feelings of worthlessness I widely used a cognitive-behavioral approach based on the Compassion Focused Therapy (Gilbert, 2012). At the end of the treatment the patient reported a significant reduction of the depression and anxiety symptoms, with important repercussions in terms of mood, of the physical well-being and of the family relationships. The results were kept three months apart as can be seen from the follow up, showing that the cognitive-behavioral therapy of third wave, in particular ACT, it is widely effective in the treatment of depressive and anxious symptoms. | Abstract (Italian): | In questo studio di caso si propone la concettualizzazione e il trattamento cognitivo-comportamentale, secondo il modello dell'Acceptance and Commitment Therapy (ACT), in una paziente di 21 anni con sindrome ansioso-depressiva, con una storia familiare caratterizzata da trascuratezza. Il trattamento ha previsto l'utilizzo della Matrice ACT (Polk et al., 2016), quale strumento per la concettualizzazione del caso e la terapia con l'obiettivo di sostenere la paziente nell'incremento della flessibilità psicologica, al fine di vivere una vita improntata verso i suoi valori. Contemporaneamente è stata avviato un training base di Mindfulness (Preziosi, 2016; Kabat-Zinn et al., 2010) per aumentare la disponibilità della paziente ad accogliere le sensazioni somatiche e i pensieri disturbanti. Poiché la paziente presentava vissuti autosvalutanti è stato ampiamente utilizzato un approccio cognitivo-comportamentale orientato alla Terapia Focalizzata sulla Compassione (Gilbert, 2012). Al termine del trattamento la paziente ha riportato una notevole remissione della sintomatologia ansiosa e depressiva, con importanti ripercussioni sul tono dell'umore, sul benessere fisico e sulle relazioni con i familiari. I risultati sono stati mantenuti a distanza di tre mesi così come si evince dal follow-up, evidenziando come la terapia cognitivo-comportamentale di terza onda, in particolare l'ACT, si riveli molto efficace nel trattamento dei sintomi ansiosi e depressivi.
