| Abstract: | Perfusion imaging (either CT or MR) which has been widely used in recent randomized controlled clinical trials of mechanical thrombectomy (MT) [[1]] and IVT in the extended time window [[2]] can promptly identify stroke mimics and optimize therapeutic management. CT perfusion showed areas of Tmax>4 s prolongation in the left parietal and left frontal regions while CT angiography was normal (Supplementary Fig. Initial CT/CT angiography and CT perfusion (supplemental figure) postprocessed with RAPID software (IschemaView, Menlo Park, CA) demonstrated a bilateral frontal and right parietal hypoperfusion which was considered discordant with the clinical examination, and the patient was admitted to the stroke unit.
