Abstract: The article informs that there are convincing pathological and epidemiological arguments for the hypothesis that the inflammatory process in multiple sclerosis (MS) is responsible for axonal loss, leading to irreversible deficits. This is a strong argument for using immunosuppressive agents in MS. However, there is also accumulative evidence that the pathogenesis of disability progression is a two-stage disease, involving inflammation and neurodegeneration. Immunosuppressive therapy may have a role in MS, but the timing of such therapy is important.
