Abstract: The article highlights the initiative of Rush University System for Health's Social Work and Community Health Department in promoting birth equity with team-based health and social care. Topics discussed include development of the Center for Health and Social Care Integration (CHaSCI) care model, the Birth Equity initiative launched by the Illinois Perinatal Quality Collaborative in Chicago, Illinois, and impact of the CHaSCI care model on patient outcomes.
