Abstract: The article focuses on the critical role of interoperability—the capacity for different health and social care systems to exchange and use information in real time—in improving coordinated care across diverse practice settings. It highlights how interoperability enhances social work by enabling seamless data sharing among providers, reducing administrative burdens, preventing care gaps, and supporting closed-loop referrals. Two key initiatives advancing interoperability standards are the Post-Acute Care InterOperability Project (PACIO), which addresses clinical transitions using the International Classification of Functioning (ICF), and the Gravity Project, which standardizes coding for social determinants of health (SDOH). The integration of interoperable systems is positioned as essential for demonstrating social work’s impact on health outcomes, advancing value-based care, and promoting health equity.
