Abstract: This article focuses on a case of brachial plexopathy, a rare complication following anterior cervical spine surgery, characterized by severe neuropathic pain, motor weakness, and sensory deficits in the right upper limb of a 43-year-old woman. High-resolution ultrasonography (US) was utilized to identify swelling of the C5, C6, and C7 nerve roots in the supraclavicular region, correlating with electrodiagnostic findings of (sub)acute axonal injury predominantly affecting the upper and middle trunks of the brachial plexus. The study highlights the diagnostic and interventional value of US, including sono-palpation to reproduce pain and US-guided perineural steroid injection, which contributed to partial pain relief. The report underscores the challenges in early diagnosis of postoperative brachial plexopathy and the importance of imaging combined with electrodiagnostic studies for precise localization and management.
