Abstract: Introduction: The alveolar bone has the natural tendency to become narrower after tooth extraction, thus losing its original dimension, because the bone quickly resorbs, resulting in loss of bone volume until the first 4–6 months. Among all the platelet concentrates, platelet-rich fibrin (PRF) has been shown to possess fibrin, leukocyte, and a variety of growth factors that could promote wound healing. Hence, the present study aims to describe the clinical and radiographic dimensional changes after 4 months of tooth extraction in combination with and without leukocyte and PRF (L-PRF) application. Materials and Methods: A split-mouth study design with 18 patients requiring bilateral symmetrical extraction with adjacent teeth intact was selected and randomized to two groups: (1) atraumatic extraction and socket filling with L-PRF and (2) controls with simple atraumatic extraction without socket filling. Baseline and follow-up were performed at 4 months, and a clinicoradiographic analysis was done. Results: There was a statistically significant increase in the buccolingual dimension at 4 months in the L-PRF socket-treated group. The mesiodistal width was statistically significant when compared to baseline (P = 0.034, Z = −2.121) in the control group. Furthermore, in the test group, the buccolingual width of the socket was statistically significant (P = 0.014, Z = −2.449) when compared with the baseline. Radiographically, an increase in the height of the socket is appreciated in both the groups, but no statistical significance was seen in between the groups. Conclusion: Within the limitations of the present study, it can be concluded that the use of L-PRF may not significantly influence the mesiodistal width, height, or overall width of the extraction socket; however, it appears to be beneficial in enhancing the buccolingual dimension, thus helping in convenient implant planning and placement.
