Abstract: Purpose: To assess the prevalence and risk factors of abnormal vertebral artery flow in subjects with carotid stenosis and controls. Methods: Flow in the vertebral arteries was assessed by Duplex scanning and color-Doppler in 225 subjects with carotid artery stenosis and 221 controls matched by age and sex who participate in a population-based study on carotid stenosis. Flow was categorized as orthograde (=normal), retrograde or absent (not detectable), and association to risk factors and clinical events was analyzed. Results: Abnormal vertebral flow (absent/retrograde) was seen in 45 of the 446 subjects. It was more common in men than in women (14.0% vs. 4.4%, p=0.001), and in individuals with carotid stenosis compared to controls (13.3% vs. 6.7%, p=0.02). In univariate analysis, abnormal vertebral flow was associated with male gender, presence of carotid stenosis, diabetes, treated hypertension, HDL cholesterol (inverse), and body mass index, but not with age, smoking, total cholesterol, triglycerides, or fibrinogen. In multivariate analysis, abnormal vertebral flow was independently associated with male gender (p=0.002), hypertension (p=0.0002), and diabetes (p=0.02), and in controls only also with HDL cholesterol (inverse). Absence of vertebral flow was associated with previous stroke in the vertebro-basilar territory (p=0.0007). There was no significant association between retrograde flow and clinical events in the vertebro-basilar territory. Conclusion: The prevalence of abnormal flow in the vertebral artery was 13.3% in subjects with carotid stenosis and 6.7% in subjects without carotid artery stenosis, although the association observed in univariate analysis between body mass index and no detectable flow indicates that some misclassification occurred.
