Abstract (English): The article analyzes social and cultural determinants that impact on access, adherence and timely follow-up of antiretroviral treatment of the Tseltal and Chol population with hiv in Chiapas and strategies of care and accessibility to treatment of support networks and health personnel. It contributes to the scarce existing research on non-adherence to antiretroviral treatment among indigenous peoples in Mexico and Latin America. The methodology is qualitative, based on a case study in an hiv care clinic (saih), during the period 2017-2018, with health personnel, users and family members, and is complemented with descriptive quantitative data on mortality and differential vertical transmission. The results demonstrate the impact on this population of the centralization of treatment in the saih, of the economic difficulties for travel, of the stigma and discrimination existing in their communities, and of the organizational culture of the saih. Strategies regarding care and/or pluralism of care are also identified. It is urgent to generate knowledge and specific measures from public policies and their implementation. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): El artículo analiza determinantes sociales y culturales que impactan en el acceso, adherencia y seguimiento oportuno del tratamiento antirretroviral de la población tseltal y chol con vih en Chiapas y estrategias de cuidados y de accesibilidad al tratamiento de las redes de apoyo y del personal de salud. Aporta a las escasas investigaciones existentes respecto a la no adherencia al tratamiento antirretroviral de los pueblos indígenas en México y América Latina. La metodología es cualitativa, basada en un estudio de caso en una clínica de atención al vih (saih), durante el periodo 2017-2018, con personal de salud, usuarios y familiares, y se complementa con datos cuantitativos descriptivos sobre mortalidad y transmisión vertical diferenciada. Los resultados demuestran el impacto en esta población de la centralización del tratamiento en el saih, de las dificultades económicas para el viaje, del estigma y la discriminación existente en sus comunidades y de la cultura organizacional del saih. Se identifican también estrategias respecto al cuidado y/o al pluralismo asistencial. Es apremiante generar conocimiento y medidas específicas desde las políticas públicas y su implementación.
