Particularities of materials used during the root canal treatment such as irritant solutions, instrumentation systems, intra-canal medication, obturation sealers, and restorative protocol are aspects that are closely linked to treatment outcomes and can influence the repair of periapical changes. However, there are individual aspects that should be addressed during the development of Endodontic materials in order to promote higher rates of success. In this sense, this study aimed to investigate the molecular aspects involved in persistent apical periodontitis and their impacts in the individual response after root canal treatment. Patients with pulpal necrosis and apical periodontitis installed were submitted to root canal treatment with a standardized protocol of materials, including regarding the restorative procedures. After one year, they were called for a follow-up, which included 212 patients in this study. Of these, 101 patients had clinical and radiographic signs and symptoms indicative of persistent apical periodontitis and 111 indicative of lesion repair. Saliva samples were collected as a source of genomic DNA, which was genotyped for soluble tumor necrosis factor-alpha mediator (rs1800629), TNF-α alpha and beta receptors (rs1800693 and rs1061622), and suppressor of cytokine signaling 1 (rs243327 and rs33977706) by real-time PCR. All genetic polymorphisms were in Hardy-Weinberg equilibrium. The frequency of genotypes and alleles was evaluated using odds ratio, chi-square test, or Fisher's exact tests (p0.05). In SOCS-1 there was no association between genotype distribution (rs243327 p=0.0788/rs33977706 p=0.0789) nor between allelic distribution (rs243327 p=0.12/rs33977706 p=0.0889). However, for both there was an association in the recessive model analysis for rs243327 (p = 0.0299) and rs33977706 (p = 0.031). Thus, these results reinforce the influence of host response to root canal treatment, since genetic factors are associated with the development and repair of apical periodontitis, suggesting possible therapeutic targets interesting for clinical practice, which should be approached during the development of new materials.
