Abstract (English): Objective: this study aimed to analyze the data of seven in-patients with the diagnosis of paracoccidioidomycosis, in the state of Paraiba and in the nearby state of Rio Grande do Norte, localized in the northeast of Brazil. Method: the cases were divided in autochthons and not autochthons according to the location where the patient acquired the disease, in other states of Brazil, with a recognized high incidence or with lower incidence as well as in the state of Paraiba. Results: besides it is emphasized the polymorphism of the disease, the variety of symptoms as well as the expressive number of other diseases that make differential diagnosis with this systemic mycosis difficulty, for instance: tuberculosis, neoplasia, inflammatory diseases of the bowels and other systemic mycosis. Conclusions: we showed the geographic position of the cases in our state, according to the city of origin of each in-patient and finally, we try to alert to our medical community the importance of diagnosis of this disease in our region. Abstract (Portuguese): Objetivo: analisar dados de sete pacientes procedentes de várias regiões do Estado da Paraíba nos quais foi estabelecido o diagnóstico de paracoccidioidomicose. O diagnóstico foi efetuado no período de agosto de 1986 a outubro de 2009, no Serviço de Doenças Infecciosas do Hospital Universitário Lauro Wanderley da Universidade Federal da Paraíba. Em um desses casos, o diagnóstico foi estabelecido no Estado do Rio Grande do Norte, em paciente residente e procedente da Paraíba. Métodos: os casos foram caracterizados como importados ou autóctones, considerando-se o fato de os mesmos terem sido adquiridos em outros Estados da Federação com elevada incidência da enfermidade ou, em nosso Estado, com história pregressa de baixa incidência da citada doença. Enfatizamos o polimorfismo do quadro clínico em virtude da variedade dos sintomas que a doença pode apresentar e o expressivo número de enfermidades que fazem o diagnóstico diferencial com essa micose, em particular as neoplasias, a tuberculose, outras micoses sistêmicas, as doenças inflamatórias intestinais etc. Indicamos a posição geográfica de onde os pacientes foram originariamente encaminhados. Resultados: estabelecimento de correlação das atividades laborais dos sujeitos com fatores de risco para paracoccidioidomicose e, por meio de tabelas, mostrar os sintomas e o comprometimento dos vários órgãos em cada um dos enfermos. Conclusão: necessidade de informar a comunidade, em particular médicos, dentistas e outros profissionais da saúde, sobre a importância de se incluir a paracoccidioidomicose no diagnóstico diferencial de doenças agudas e crônicas que se caracterizam, sobretudo, pelo comprometimento pulmonar e cutâneo-mucoso.
