Abstract (English): Point-of care ultrasound (POCUS) has gained great importance in the perioperative period to establish the etiology of the undifferentiated shock, guide and monitor the management. The focused echocardiography guided by goals has four basic views: parasternal long axis, parasternal short axis at the level of the papillary muscles, subcostal 4 chambers and subcostal longitudinal axis of the inferior vena cava that generate basic information for the anesthesiologists and intensivists to direct the initial management. The qualitative examination requires appropriate training, and it does not replace the one performed by the echocardiographer, but has shown timely management, effectiveness in knowing the cause of the shock and possibility of real-time follow-up to the therapy instituted. Abstract (Spanish): El uso del ultrasonido enfocado (POCUS) ha cobrado gran importancia en el perioperatorio para establecer la etiología de un shock no diferenciado, guiar y monitorizar el manejo del mismo. La ecocardiografía enfocada por metas consta de cuatro vistas básicas: paraesternal eje largo, paraesternal eje corto a nivel de los músculos papilares, subcostal 4 cámaras y subcostal eje longitudinal de la vena cava inferior que generan información fundamental para que el anestesiólogo e intensivista dirijan el manejo inicial. El examen cualitativo requiere de entrenamiento apropiado y, no reemplaza el realizado por el ecocardiografista, pero ha demostrado oportunidad en el manejo del paciente, efectividad en conocer la causa del shock y posibilidad de seguimiento en tiempo real a la terapia instaurada.
