| Abstract (English): |
Serious allergic reactions to vaccines are rare and difficult to predict. An allergic reaction can be defined as an idiosyncratic reaction caused by an immune mechanism. The World Allergy Organization (WAO) has recommended categorizing immune reactions to drugs (including vaccines) based on when symptoms appear. Patients who experience vaccine reactions that may have represented immediate IgEmediated allergic reactions should be referred to an allergist for evaluation, if possible. Studies have shown that puncture and intradermal tests with Pfizer's full-strength COVID-19 mRNA vaccine are non-irritating. Negative skin test results would indicate that the reaction may not have been mediated by IgE, and administration of the second dose in the usual manner under observation may be considered. The administration of second doses in these circumstances requires shared decision making with the patient, weighing the risk of a vaccine reaction against the risk of remaining inadequately vaccinated against COVID-19. As reações alérgicas graves às vacinas são raras e difíceis de prever. Uma reação alérgica pode ser definida como uma reação idiossincrática causada por um mecanismo imunológico. A Organização Mundial de Alergia (WAO) recomendou categorizar as reações imunológicas a drogas (incluindo vacinas) com base no momento do aparecimento dos sintomas. Os pacientes que apresentaram reações vacinais que possam ter representado reações alérgicas de tipo imediato mediadas por IgE devem ser encaminhados a um alergista para avaliação, se possível. Estudos demonstraram que os testes de puntura e intradérmicos com a vacina COVID-19 de mRNA da Pfizer de força total não são irritantes. Os resultados negativos do teste cutâneo indicariam que a reação pode não ter sido mediada por IgE, e pode-se considerar a administração da segunda dose da maneira usual sob observação. A administração de segundas doses nessas circunstâncias requer tomada de decisão compartilhada com o paciente, pesando o risco de uma reação à vacina contra o risco de permanecer vacinado de forma inadequada contra o COVID-19.
