| Abstract (English): |
We describe the case of a 33-year-old woman with three years of vulvodynia associated with recurrent outbreaks of genital herpes infection. The physical examination stands out neuropathic characteristics of the perineal pain, with burning, pricking sensation and allodynia, predominantly to the left side of the vulva. The neurophysiological study is normal. After failure of treatment with oral gabapentine and topic lidocaine with llanten (plantago major: anti-inflammatory effect), an Eco guided infiltration of corticoids and local anesthetic on the pudendal nerve is performed, this being effective in controlling symptoms. Thus, we finally performed Pulsed Radiofrequency Stimulation on the left pudendal nerve, with a successful relief of the pain that lasted five months approximately. [ABSTRACT FROM AUTHOR] |

Presentamos el caso clínico de una mujer de 33 años con clínica de vulvodinia de tres años de evolución, asociado a infección recurrente por herpes genital. A la exploración física destaca dolor de características neuropáticas en la zona genital (labio externo izquierdo de la zona vulvar) en forma de quemazón, pinchazos y alodinia. El estudio neurofisiológico no muestra hallazgos patológicos. Tras fracaso del tratamiento farmacológico con gabapentina oral y aplicación tópica de llanten (planta con efecto antiinflamatorio) y lidocaína al 2%, se realiza infiltración ecoguiada del nervio pudendo izquierdo con anestésico local y corticosteroides. Ante la disminución de su sintomatología, se realiza radiofrecuencia pulsada sobre el nervio pudendo izquierdo, tras lo cual se constata una mejoría clínica significativa del dolor, con una duración del efecto de aproximadamente cinco meses.
