Introduction: Myelopathy is the general term that describes any disease that affects the spinal cord. Objective: To clinically characterize patients with myelopathies. Methods: An observational, descriptive, retrospective study was carried out in patients admitted to the neurology service at Hermanos Ameijeiras Surgical Clinical Hospital from 2005 to 2020. Results: Patients in the fifth decade of life and females predominated (25.6% and 51.5%, respectively). The chronic installation form (55.6%) was the most common, paraparesis/paraplegia (60.8%), anterolateral syndrome (41.9%) and the absence of sphincter involvement (55.9%) were those of higher frequency. Multiple sclerosis (13.7%), motor neuron diseases (12.9%), and spondylotic myelopathy (11.9%) were the leading causes of spinal cord damage among study subjects. Medical treatment was the most used. At the time of admission, moderate disability predominated (51.2%), which was maintained at discharge with the same classification on the scale of the American Association of Spine Injuries 53.4%), followed by those who improved (35.6%). The etiology with the best evolution at discharge was multiple sclerosis (10.3%) and the worst was extradural tumors (3.9%). Conclusions: Paraparesis, anterolateral spinal syndrome and the form of chronic installation characterize the patients with myelopathies included in the study, whose main etiology is multiple sclerosis and conservative treatment is the most used. Prompt and timely action is required from the first moment spinal cord involvement is diagnosed to avoid disability and serious financial losses that impact the quality of life of patients and their families. Introducción: Mielopatía es el término general que describe cualquier enfermedad que afecta a la médula espinal. Objetivo: Caracterizar clínicamente los pacientes con mielopatías. Métodos: Se realizó un estudio observacional, descriptivo, retrospectivo en pacientes ingresados en el servicio de neurología del Hospital Clínico Quirúrgico "Hermanos Ameijeiras" entre los años 2005 al 2020. Resultados: Predominaron los pacientes en la quinta década de vida y del sexo femenino (25,6 % y 51,5 %, respectivamente). La forma de instalación crónica (55,6 %) fue la más común, la paraparesia/paraplejia (60,8 %), el síndrome anterolateral (41,9 %) y la ausencia de afectación esfinteriana (55,9 %) los de mayor frecuencia de presentación. La esclerosis múltiple (13,7 %), las enfermedades de motoneurona (12,9 %) y la mielopatía espondilótica (11,9 %) fueron las principales causas de daño en la médula espinal entre los sujetos del estudio. El tratamiento médico fue el más utilizado. En el momento del ingreso predominó la discapacidad moderada (51,2 %) lo que se mantuvo al egreso con igual clasificación en la escala de la Asociación Americana de Lesiones de la Columna Vertebral (53,4 %), seguido de los que mejoraron (35,6 %). La etiología con una mejor evolución al egreso fue la esclerosis múltiple (10,3 %) y la de peor los tumores extradurales (3,9 %). Conclusiones: La paraparesia, el síndrome espinal anterolateral y la forma de instalación crónica caracterizan a los pacientes con Mielopatías incluidos en el estudio, cuya etiología principal es la esclerosis múltiple y el tratamiento conservador el más empleado. Se requiere una acción rápida y oportuna desde el primer momento que se diagnostica una afectación de la médula espinal para evitar discapacidad y prejuicios económicos graves que impacten en la calidad de vida de pacientes y sus familiares.
