Abstract (English): The latest clinical trials of stroke patients undergoing mechanical thrombectomy v/s better medical treatment showed favorable results even in patients with an extensive ischemic core. A recent meta-analysis showed that it was even beneficial for patients with extensive ischemia and up to 24 hours since the last time seen well (Saraj, 2023). In this article, we review the different clinical trials of thrombectomy, the pathophysiology of ischemia and propose the astrocyte and the glymphatic system as the focus on the effects of treatment. We propose the need to change the paradigm of current treatment in patients with extensive ischemic core, as well as the need to consider these changes in the financing and organization of a national thrombectomy network by government authorities. Abstract (Spanish): Los últimos ensayos clínicos de pacientes con ataque cerebrovascular sometidos a trombectomía mecánica v/s mejor tratamiento médico, mostraron resultados favorables incluso en pacientes con un "core" isquémico extenso. En un metaanálisis reciente se mostró que incluso era beneficioso para pacientes con una isquemia extensa y hasta 24 hrs desde la última vez visto bien (Saraj, 2023). En este artículo revisamos distintos ensayos clínicos que evalúan la trombectomía, la fisiopatología de la isquemia y proponemos al astrocito y el sistema glinfático, como foco principal en los efectos del tratamiento. Planteamos la necesidad de cambiar el paradigma del tratamiento actual en pacientes con "core" isquémico extenso, a su vez la necesidad de considerar estos cambios en el financiamiento y la organización de una red nacional de trombectomía por parte de las autoridades gubernamentales.
