| Abstract (English): | Chronic post-thoracotomy pain (CPTP) is a common complication following thoracic surgery, affecting 30-50% of the patients. It is characterized by persistent pain lasting beyond three months, metameric distribution, and neuropathic features. The management of CPTP, particularly in refractory cases, remains a major clinical challenge. We present the case of a patient with refractory CPTP who, after multiple unsuccessful therapeutic strategies, underwent the implantation of a dorsal root ganglion (DRG) neurostimulation system, achieving an 80% pain reduction and a significant improvement in quality of life. This technique enables precise, metameric modulation of nociceptive transmission, effectively targeting pain in the affected area while reducing reliance on analgesic medications. With growing clinical evidence supporting its efficacy, DRG neurostimulation is emerging as a promising and effective therapeutic option for localized neuropathic pain, potentially redefining the management of refractory CPTP. | Abstract (Spanish): | El dolor crónico postoracotomía (DCPT) es una complicación frecuente de la cirugía torácica, con una prevalencia del 30-50%. Se caracteriza por ser un dolor que aparece o se agrava tras una cirugía o lesión, persiste más de tres meses en la zona afectada o su inervación y no tiene causas preexistentes. Presentamos el caso de una paciente con DCPT intenso, resistente a múltiples terapias convencionales, que experimentó una reducción del dolor del 80% y una mejora funcional significativa tras la implantación de un sistema de neuroestimulación del ganglio de la raíz dorsal (GRD). Este enfoque permite una modulación precisa de la transmisión nociceptiva a nivel metamérico, controlando el dolor en la región afectada y reduciendo la necesidad de medicación analgésica. La creciente evidencia respalda la estimulación del GRD como una opción terapéutica eficaz y prometedora en el manejo del dolor neuropático localizado, mejorando el pronóstico en pacientes con DCPT refractario.
