Abstract (English): Sepsis remains a major cause of morbidity and mortality and continues to pose substantial diagnostic challenges in its early stages. Monocyte Distribution Width (MDW), a parameter automatically integrated into complete blood counts by next generation haematology analysers, has emerged as a promising biomarker. This study evaluates the diagnostic performance of MDW compared with traditional markers (C-reactive protein and leukocyte count) and clinical scoring systems (SOFA and NEWS) in a cohort of critically ill patients presenting to a hospital emergency department, with the aim of distinguishing those with infection from those whose severity was attributable to other causes. Abstract (Spanish): La sepsis continúa siendo una causa relevante de morbimortalidad con dificultades diagnósticas relevantes en fases precoces. El parámetro MDW (Monocyte Distribution Width), integrado automáticamente en hemogramas de analizadores de última generación, ha emergido como un biomarcador prometedor. Este estudio analiza el rendimiento diagnóstico de MDW comparado con otros marcadores clásicos (proteína C reactiva y leucocitos) y escalas clínicas (SOFA y NEWS) en una cohorte de pacientes con criterios de gravedad atendidos en un Servicio de Urgencias Hospitalario para identificar pacientes con infección frente a los que presentan otro origen de su situación de gravedad.
