Abstract (English): We present the case of a 20-year-old female patient with neurological symptoms whose magnetic resonance imaging (MRI) revealed a left frontoinsular mass. Multiparametric MRI - including diffusion-weighted imaging, diffusion tensor imaging), resting-state functional MRI, and ALPS (along the perivascular space) index - suggested an IDH (isocitrate dehidrogenase) mutant, 1p/19q non-codeleted glioma. Histopathological and molecular analyses confirmed the imaging-based hypothesis. This case highlights the power of advanced MRI techniques in the preoperative molecular characterization of gliomas. Abstract (Spanish): Se presenta el caso de una paciente de 20 años con síntomas neurológicos, cuya resonancia magnética (RM) reveló una masa en la región frontoinsular izquierda. La evaluación mediante RM multiparamétrica-incluyendo secuencias de difusión, tensores de difusión, RM funcional en estado de reposo e índice ALPS (along the perivascular space)-sugirió un glioma mutado para IDH (isocitrate dehidrogenase) y sin codeleción 1p/19q. Los estudios histopatológicos y moleculares confirmaron la sospecha diagnóstica. Este caso resalta el valor de las técnicas avanzadas de RM en la caracterización molecular preoperatoria de los gliomas. [ABSTRACT FROM AUTHOR]
