| Abstract (English): |
The treatment of obesity is a multidisciplinary effort that involves the patient, her/his family, different health professionals, and must consider her/his environment. The objectives are to improve nutritional status, reduce excess weight and related complications, as well as to promote the maintenance of healthy lifestyle habits to prevent the development of chronic non-communicable diseases in adults. In this article, we present nutritional interventions, a fundamental part of the treatment, incorporating the patient and her/his family in decision-making and education. Due to the benefits of physical activity, its promotion and reduction of sedentary hours are indications that should always be present, so the type, quantity, intensity, and recommended frequency are described. In relation to psychosocial management, early referral to psychology is suggested in order to investigate psychosocial comorbidities and evaluate the need for psychiatric support and/or treatment. All patients and especially adolescents with obesity may experience lower self-esteem, body image alterations, and dysfunctional eating behaviors, thereby altering their quality of life in several dimensions. Motivational interviewing is used on the basis that the individual her/himself discovers and explores reasons to change and, from this reflection, generates the therapeutic alliances necessary to bring about lifestyle changes. Finally, severe obesity is addressed, which is associated with greater morbidity, deterioration of quality of life, and premature mortality, and therefore requires intensive study and treatment. El tratamiento de la obesidad es un trabajo multidisciplinario que considera al paciente, su familia, diferentes profesionales de la salud y debe conocer su entorno. Los objetivos son mejorar el estado nutricional, disminución del exceso de peso y de las complicaciones derivadas, y promover la mantención de los hábitos de vida saludable para prevenir el desarrollo de las enfermedades crónicas no transmisibles del adulto. En este artículo presentamos las intervenciones nutricionales, parte fundamental del tratamiento, incorporando en las decisiones y educación al paciente y su familia. Debido al beneficio que la práctica de actividad física tiene, su promoción y disminución de las horas sedentarias son indicaciones que siempre deben estar presentes, por lo que se describe el tipo, cantidad, intensidad y frecuencia recomendada. En relación con el manejo psicosocial, se sugiere derivar precozmente a psicología para pesquisar comorbilidades psicosociales y evaluar necesidad de apoyo y/o tratamiento psiquiátrico; todos los pacientes y especialmente adolescentes con obesidad pueden experimentar mayor compromiso de autoestima, alteraciones de imagen corporal y conductas alimentarias disfuncionales, alterándose la calidad de vida en varias dimensiones. La entrevista motivacional se utiliza basándose en que el mismo individuo descubra y explore razones para cambiar y a partir de esta reflexión generar las alianzas terapéuticas necesarias para producir cambios de estilos de vida. Finalmente se aborda la obesidad severa, la cual se asocia a mayor morbilidad, deterioro de la calidad de vida y mortalidad prematura, por lo cual requiere estudio y tratamiento intensivo.
