Abstract (English): OBJECTIVE: To describe the most frequent anatomical variants of the anterior ethmoidal artery in adults through the analysis of computed tomography scans performed at a tertiary-level hospital. MATERIALS AND METHODS: A descriptive, cross-sectional, and retrospective study analyzing computed tomography scans of patients from the Regional Hospital General Ignacio Zaragoza, ISSSTE, from September 1st to December 31, 2024. The evaluation included the presence of supraorbital ethmoidal cells, the Keros index, the anterior ethmoidal canal, and the localization of the anterior ethmoidal artery. RESULTS: There were studied 97 computed tomography scans. Supraorbital ethmoidal cells were present in 32% of cases. Grade II was the most common in both the Keros index (73%) and the anterior ethmoidal canal (52%). A 29% asymmetry was observed between left and right Keros indices. The predominant location of the anterior ethmoidal artery was between the second and third lamellae (69%). CONCLUSIONS: Computed tomography imaging allows for preoperative evaluation of the anatomy and variants of the anterior ethmoidal artery in patients undergoing endoscopic sinus surgery, with the aim of reducing associated complications. Abstract (Spanish): OBJETIVO: Describir las variantes anatómicas más frecuentes de la arteria etmoidal anterior en adultos mediante el análisis de tomografías computadas tomadas en un hospital de tercer nivel. MATERIALES Y MÉTODOS: Estudio descriptivo, transversal y retrospectivo, en el que se revisaron tomografías de pacientes del Hospital Regional General Ignacio Zaragoza, ISSSTE, del 1 de septiembre al 31 de diciembre de 2024. Se evaluó la existencia de celdillas etmoidales supraorbitarias, el índice de Keros, el canal etmoidal anterior y la localización de la arteria etmoidal anterior. RESULTADOS: Se estudiaron 97 tomografías; se visualizaron celdillas etmoidales supraorbitarias en el 32% de los casos. El grado II fue el más común en el índice de Keros (73%) y en el canal etmoidal anterior (52%). Además, se observó una asimetría del 29% entre los índices de Keros izquierdo y derecho. La ubicación predominante fue entre la segunda y tercera lamelas (69%). CONCLUSIONES: Con el uso de tomografía computada puede evaluarse, de manera preoperatoria, la anatomía de la arteria etmoidal anterior y sus variantes en pacientes aptos a cirugía endoscópica de senos paranasales, con el objetivo de reducir complicaciones asociadas.
