The article focuses on severe acute respiratory infections (SARI) in children under five years old, highlighting their high disease burden and mortality, especially in low-income countries. In 2023, approximately 600,000 deaths in this age group were reported as attributable to SARI, with a notable prevalence of respiratory syncytial virus (RSV). The importance of immunization strategies as key interventions to reduce child mortality is emphasized, and the success of the RSV immunization program implemented in Chile in 2024 is mentioned. Additionally, the differences in disease burden between high- and low-income countries are discussed, emphasizing the need for a research agenda that comprehensively addresses these challenges. El artículo se centra en las infecciones respiratorias agudas graves (IRAG) en niños menores de cinco años, destacando su alta carga de enfermedad y mortalidad, especialmente en países de bajos ingresos. En 2023, se reportaron aproximadamente 600,000 muertes en este grupo etario atribuibles a IRAG, con una notable prevalencia del virus respiratorio sincicial (VRS). Se subraya la importancia de las estrategias de inmunización como intervenciones clave para reducir la mortalidad infantil, y se menciona el éxito del programa de inmunización contra el VRS implementado en Chile en 2024. Además, se discuten las diferencias en la carga de enfermedad entre países de altos y bajos ingresos, enfatizando la necesidad de una agenda de investigación que aborde estos desafíos de manera integral.
