Abstract (English): The article focuses on vacuolated casts as an emerging marker in urinary sediment, especially in patients with advanced proteinuric glomerulopathies. It presents the case of a woman with diabetic nephropathy who exhibits these casts, characterized by vesicles of variable size and shape without birefringence under polarized light, differentiating them from lipid and hematic casts. Recent studies indicate that the presence of vacuolated casts is associated with nephrotic-range proteinuria and advanced chronic kidney disease, suggesting their possible prognostic value in the progression toward end-stage renal disease. Although their exact origin is unknown, hypotheses have been proposed related to vacuolated renal tubular cells, injured podocytes, or foamy macrophage cells. Precise identification of these casts requires complementary microscopic techniques to avoid diagnostic errors with clinical impact. [Extracted from the article]
Abstract (Spanish): El artículo se centra en los cilindros vacuolados como un marcador emergente en el sedimento urinario, especialmente en pacientes con glomerulopatías proteinúricas avanzadas. Se presenta el caso de una mujer con nefropatía diabética que exhibe estos cilindros, caracterizados por vesículas de tamaño y forma variable sin birrefringencia bajo luz polarizada, diferenciándolos de los cilindros lipídicos y hemáticos. Estudios recientes indican que la presencia de cilindros vacuolados se asocia con proteinuria en rango nefrótico y enfermedad renal crónica avanzada, sugiriendo su posible valor pronóstico en la progresión hacia la enfermedad renal terminal. Aunque su origen exacto es desconocido, se plantean hipótesis relacionadas con células tubulares renales vacuoladas, podocitos lesionados o células espumosas macrofágicas. La identificación precisa de estos cilindros requiere técnicas microscópicas complementarias para evitar errores diagnósticos con impacto clínico. [Extracted from the article]
