Abstract (English): Introduction: La Matanza district, located in the western zone of Buenos Aires Metropolitan Area, has a population of 1 837 774, with a high proportion of migrants from regions endemic for Chagas disease (CD). Our hospital serves as the local referral center and reactivated a dedicated CD outpatient clinic in Octo ber 2023. The objectives of this study were to describe the collected data, characterize the patient population, identify the available epidemiological information, and assess the initial response to follow-up and adherence to trypanocidal therapy. Material and methods: We conducted an observa tional, descriptive, and retrospective study of 61 patients evaluated between October 2023 and May 2024. Results: The study revealed a relatively high mean age and a high prevalence of Chagas-related cardiomy opathy, resulting in a majority of patients being ineli gible for treatment compared to those who qualified. Attempts to obtain official epidemiological data on CD in the district were unsuccessful, highlighting a substan tial degree of underreporting. Approximately 35% (21) of patients were lost to follow-up, although all prescribed treatments were completed successfully. Discussion: This study underscores the delayed sus picion and diagnosis of CD, as well as the absence of epidemiological data in a densely populated and highly migratory area. These findings reinforce the need to establish integrated services for comprehensive man agement of CD, decentralize care delivery, and promote the foundational principles of Chagas care consider CD, diagnose CD, and treat CD. Abstract (Spanish): Introducción: El distrito La Matanza, ubicado en la región oeste del conurbano bonaerense, tiene una pobla ción de 1 837 774 habitantes con un elevado porcentaje de migrantes procedentes de área endémica para enfer medad de Chagas (ECh). Nuestro hospital es el centro de referencia y en octubre de 2023 reinició actividades de un consultorio específico. Los objetivos del estudio fueron describir los datos recolectados, caracterizar la población de los pacientes atendidos, determinar la información epidemiológica disponible y evaluar respuesta inicial al seguimiento y adherencia a la terapia tripanocida. Materiales y métodos: Estudio observacional, des criptivo y retrospectivo de 61 pacientes atendidos entre octubre de 2023 y mayo de 2024. Resultados: Se observó una media de edad y tasa de cardiopatía elevadas con una mayoría de pacientes sin indicación de tratamiento versus los candidatos al mismo. Se buscaron datos estadísticos de ECh en el distrito, sin obtenerse registros, y se constató un impor tante porcentaje de subnotificación. Alrededor del 35% de los pacientes no adhirieron al seguimiento luego de la consulta inicial y todos los tratamientos indicados fueron finalizados correctamente. Discusión: Este estudio demuestra la sospecha y diagnóstico tardíos de ECh y la ausencia de datos en un sitio con alta densidad poblacional y migratoria, reforzando la necesidad de establecer servicios para el manejo integral de esta enfermedad, descentralizando su abordaje y promoviendo los principios de cuidados básicos pensar ECh, diagnosticar ECh y tratar ECh.
