Abstract (English): Objective: To determine the antimicrobial susceptibility of aerobic microorganisms isolated from surgical wounds of patients with perforated acute appendicitis in a general hospital. Methods: 225 wound culture reports from the fifth postoperative day were reviewed (2017--2021). Samples were processed using the Vitek 2 automated system. Results: 143 strains were isolated from 134 positive cultures. The most frequent bacteria were Escherichia coli (60.8%) and Pseudomonas aeruginosa (21.7%). E. coli: Susceptibility was >90% for carbapenems, amikacin, colistin, piperacillin/tazobactam, and tigecycline, but <11.1% for cephalosporins, fluoroquinolones, and sulfamethoxazole/trimethoprim. Resistance was due to Extended-Spectrum Beta-Lactamase (ESBL) strains in 88.5% of cases. P.aeruginosa: Susceptible to colistin and aztreonam, but resistant to cephalosporins, carbapenems, fluoroquinolones, and aminoglycosides due to carbapenemases in 44.4% of cases. Conclusion: E. coli (88.5% ESBL+) and P. aeruginosa were the most common microorganisms. E. coli remained susceptible to carbapenems and amikacin, while P. aeruginosa showed high resistance to carbapenems. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): Objetivo: Determinar la susceptibilidad antimicrobiana de los microorganismos aeróbicos aislados de herida operatoria de pacientes con apendicitis aguda perforada en un hospital general. Material y métodos: Se revisaron los reportes de 225 cultivos de herida al quinto día postoperatorio en pacientes con apendicitis aguda perforada durante el período 2017-2021. Las muestras fueron procesadas con el sistema automatizado Vitek 2 Systems 07.01. Resultados: Se aislaron 143 cepas en 134 cultivos positivos. Las bacterias aisladas más frecuentes fueron Escherichia coli (60,8 %) y Pseudomonas aeruginosa (21,7 %). La susceptibilidad de E. coli fue mayor al 90 % para carbapenems, amikacina, colistina, piperacilina/tazobactam y tigeciclina; por el contrario, fue menor al 11,1 % para cefalosporinas, fluoroquinolonas y sulfametoxazol/trimetoprim. Esta resistencia se asocia a la presencia de cepas productoras de betalactamasas de espectro extendido (BLEE) en un 88,5 %. P. aeruginosa fue susceptible a colistina y aztreonam, y mostró resistencia a cefalosporinas, carbapenems, fluoroquinolonas y aminoglucósidos, la cual se asocia a la presencia de carbapenemasascarbapenems en un 44,4 %. Klebsiella pneumoniae fue productora de BLEE y mostró susceptibilidad similar a E. coli, aunque también fue susceptible a levofloxacino. Enterococcus faecalis fue susceptible a ampicilina, vancomicina y linezolid, y mostró resistencia a fluoroquinolonas. Staphylococcus spp. fue susceptible a vancomicina, gentamicina, levofloxacino, moxifloxacino, quinupristina/dalfopristina y linezolid. S. epidermidis (6/7) y S. warneri fueron productoras de betalactamasas. Conclusión: E. coli y P. aeruginosa fueron los microorganismos aeróbicos cultivados con mayor frecuencia. El primero, con producción de BLEE en un 88,5 %, fue susceptible a carbapenems, amikacina, colistina, piperacilina/tazobactam y tigeciclina; mientras que el segundo presentó producción de carbapenemasascarbapenems R en 44,4 %, y mostró susceptibilidad a colistina y aztreonam. Por su parte, S. epidermidis fue productora de betalactamasas en un 87,5 %, y mostró susceptibilidad a vancomicina, gentamicina y levofloxacino. [ABSTRACT FROM AUTHOR]
