Abstract (English): Background: Pigmented dermatoses are a frequent reason for pediatric dermatologic care, especially melanocytic nevi. Although they have a low risk of progression to melanoma, they may require excision if they affect the patient’s quality of life due to their location in visible sites or specific areas such as the genitals. We present the unusual case of an adolescent with a melanocytic nevus located on the genitals, who underwent surgical management. Clinical case: 11-year-old male patient who presented with a melanocytic nevus affecting the penis and with no additional abnormalities on the remainder of the physical examination. Due to the impact on quality of life, the entire pigmented plaque was excised with a 5-mm margin of healthy skin, and reconstruction of the cutaneous defect and the foreskin was performed. The histopathological study reported a completely excised intradermal melanocytic neurotized nevus, with no malignant criteria. Conclusions: Congenital melanocytic nevi can be treated with complete excision when they affect patients’ quality of life due to their location in sensitive sites, such as the genitals. Adequate reconstruction improves the patient’s prognosis, as these types of lesions often cause anxiety and depression, especially in adolescents. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): Introducción: las dermatosis pigmentadas son motivo frecuente de atención dermatológica en pediatría, especialmente los nevos melanocíticos. Aun cuando tengan un riesgo bajo de progresión a melanoma, pueden requerir extirpación si afectan la calidad de vida de los pacientes al encontrarse en sitios visibles o áreas específicas como los genitales. Se presenta el caso inusual de un adolescente con nevo melanocítico localizado en los genitales, en quien se realizó manejo quirúrgico. Caso clínico: paciente del sexo masculino de 11 años, previamente sano, quien presentó un nevo melanocítico que afectó el pene y no presentó anormalidades adicionales en el resto de la exploración física. En virtud de la afección de la calidad de vida, se realizó exéresis de toda la placa pigmentada con 5 mm de margen de piel sana y reconstrucción del defecto cutáneo, así como del prepucio. El estudio histopatológico reportó nevo melanocítico intradérmico con neurotización, totalmente extirpado, sin criterios de malignidad. Conclusiones: los nevos melanocíticos congénitos pueden tratarse con extirpación completa cuando afectan la calidad de vida de los pacientes por encontrarse en sitios sensibles, como los genitales. La reconstrucción adecuada favorece el mejor pronóstico de los pacientes, ya que este tipo de lesiones suelen causar ansiedad y depresión, sobre todo en adolescentes. [ABSTRACT FROM AUTHOR]
