BACKGROUND: Psoriasis is a chronic inflammatory disease with substantial clinical, psychological, and social impacts. In Central America and the Caribbean, its management faces significant challenges due to unequal access to treatments, the absence of regional protocols, and the lack of locally generated evidence. In response, the Central American and Caribbean Society of Dermatology led the development of a contextualized clinical practice guideline for the treatment of psoriasis. MATERIALS AND METHODS: A panel of 12 dermatologists and an independent methodological team were convened. Clinical questions were developed according to the PICO framework, addressed through systematic reviews of the literature, and the certainty of the evidence was assessed using the GRADE approach. Recommendations were developed using the evidence-to-decision framework, considering the balance of benefits and harms, patient values and preferences, and prioritizing therapeutic options available in the region. RESULTS: A total of 51 recommendations were developed, covering topical treatments, systemic therapies, and therapeutic strategies for the different clinical manifestations and anatomical locations of psoriasis, as well as special situations. Recommendations were classified as strong or conditional according to the quality of evidence. CONCLUSIONS: This guideline provides a regionally adapted tool to standardize the care of psoriasis in Central America and the Caribbean, promoting the adoption of evidence-based clinical practices tailored to local realities. ANTECEDENTES: La psoriasis es una enfermedad inflamatoria crónica con importantes repercusiones clínicas, psicológicas y sociales. En Centroamérica y el Caribe, su tratamiento enfrenta desafíos significativos debido al acceso desigual a medicamentos, la ausencia de protocolos regionales y la falta de evidencia generada localmente. En respuesta, la Sociedad Centroamericana y del Caribe de Dermatología lideró el desarrollo de una guía de práctica clínica contextualizada para el tratamiento de la psoriasis. MATERIALES Y MÉTODOS: Se conformó un panel de 12 dermatólogos y un equipo metodológico independiente. Las preguntas clínicas se desarrollaron siguiendo el formato PICO, se trataron mediante revisiones sistemáticas de la bibliografía y la certeza de la evidencia se evaluó utilizando el enfoque GRADE. Las recomendaciones se elaboraron aplicando el marco de evidencia para la toma de decisiones, considerando el equilibrio entre beneficios y riesgos, los valores y preferencias de los pacientes, y priorizando las opciones terapéuticas disponibles en la región. RESULTADOS: Se formularon 51 recomendaciones que abarcan tratamientos tópicos, terapias sistémicas y estrategias terapéuticas para las diferentes manifestaciones clínicas y localizaciones anatómicas de la psoriasis, así como para situaciones especiales. Las recomendaciones se clasificaron como fuertes o condicionales según la calidad de la evidencia. CONCLUSIONES: Esta guía constituye un método adaptado al contexto regional para estandarizar la atención de la psoriasis en Centroamérica y el Caribe y promueve la adopción de prácticas clínicas basadas en la evidencia y ajustadas a las realidades locales.
