Introduction. Non-communicable diseases represent one of the leading causes of morbidity and mortality in Mexico, significantly affecting family and social dynamics. In this context, primary caregivers assume a fundamental role in the continuous care of individuals with functional dependence, which may lead to physical, emotional, social, and economic burden. Objective. To analyze the level of caregiver burden and associated variables among primary caregivers in two general hospitals in the state of Tamaulipas. Methods. A descriptive, cross-sectional, and comparative study was conducted with 155 primary caregivers from two regional hospitals in Tamaulipas. A sociodemographic questionnaire and the Zarit Burden Interview were administered to assess caregiver burden. Kolmogorov–Smirnov tests were used to assess normality, and Student’s t test, χ² test, ANOVA, and Spearman’s rho were applied for correlation analyses. Data were analyzed using SPSS version 26. Results. The sample consisted predominantly of women (56.1%), with a median age of 38 years. Overall caregiver burden was low in both hospitals (mean = 35.22 ± 16.58 and 34.00 ± 15.47, respectively). Significant positive correlations were found between the total Zarit score and symptoms such as fatigue (ρ = 0.558), pain (ρ = 0.477), sleep problems (ρ = 0.456), and time devoted to caregiving (ρ = 0.217). No statistically significant differences were identified according to sex, employment status, or rest periods. The instrument demonstrated high reliability (Cronbach’s alpha = 0.935). Conclusions. The findings highlight the need to alert healthcare teams to the signs and symptoms that increase the perception of caregiver burden as part of comprehensive patient care. Furthermore, the importance of incorporating an integral approach within health services that considers both the patient and the caregiver within the healthcare system is emphasized. Introducción. Las enfermedades no transmisibles representan una de las principales causas de morbilidad y mortalidad en México, afectando significativamente la dinámica familiar y social. En este contexto, las personas cuidadoras primarias asumen un rol fundamental en la atención continua de personas con dependencia funcional, lo que puede generar una sobrecarga física, emocional, social y económica. Objetivo. Analizar el nivel de sobrecarga de las personas cuidadoras primarias y las variables asociadas en dos hospitales generales del estado de Tamaulipas. Métodos. Se realizó un estudio descriptivo, transversal y comparativo en 155 cuidadores primarios de dos hospitales regionales de Tamaulipas. Se aplicó una cédula sociodemográfica y el test de Zarit para evaluar la sobrecarga. Se utilizaron pruebas de Kolmogórov-Smirnov para la normalidad, t de Student, χ², ANOVA y Rho de Spearman para correlaciones. El análisis se realizó en SPSS v.26. Resultados. La muestra estuvo conformada mayoritariamente por mujeres (56.1%), con una mediana de edad de 38 años. La sobrecarga general fue baja en ambos hospitales (Media = 35.22 ± 16.58 y 34.00 ± 15.47, respectivamente). Se hallaron correlaciones positivas y significativas entre la puntuación total del test de Zarit y síntomas como fatiga (ρ = 0.558), dolor (ρ = 0.477), problemas para dormir (ρ = 0.456) y tiempo dedicado al cuidado (ρ = 0.217). No se identificaron diferencias estadísticamente significativas en relación con sexo, condición laboral ni periodos de descanso. El instrumento mostró alta confiabilidad (α de Cronbach = 0.935). Conclusiones. Nuestros hallazgos subrayan la necesidad de alertar al equipo de salud sobre los signos y síntomas que incrementan la percepción de sobrecarga como parte del cuidado integral al paciente. Asimismo, se destaca la importancia de incluir en los servicios de salud de un abordaje integral que contemple tanto al paciente como al cuidador dentro del sistema de atención en salud.
