Abstract (English): The article focuses on the relevance and application of auscultatory percussion, specifically the patellar-pubic percussion test (PPPT), for the diagnosis of occult hip fractures that are not evident on initial X-rays. The PPPT is a non-invasive clinical maneuver that compares the sound transmitted between both limbs to detect bone discontinuities, showing high sensitivity (0.96) and specificity (0.86) according to scientific evidence. Additionally, this technique can be extended to other bone fractures and is useful in emergency settings as a low-cost tool to guide the need for more advanced imaging studies. However, it has limitations in patients with prostheses or conditions that alter sound transmission, so its systematic incorporation into clinical evaluation and its teaching in medical training are recommended. Abstract (Spanish): El artículo se centra en la relevancia y aplicación de la percusión auscultatoria, específicamente el test de percusión patelar-púbica (TPPP), para el diagnóstico de fracturas ocultas de cadera, que no son evidentes en radiografías iniciales. El TPPP es una maniobra clínica no invasiva que compara el sonido transmitido entre ambas extremidades para detectar discontinuidades óseas, mostrando alta sensibilidad (0.96) y especificidad (0.86) según evidencia científica. Además, esta técnica puede extenderse a otras fracturas óseas y es útil en emergencias como herramienta de bajo costo para orientar la necesidad de estudios de imagen más avanzados. Sin embargo, presenta limitaciones en pacientes con prótesis o condiciones que alteran la transmisión del sonido, por lo que se recomienda su incorporación sistemática en la evaluación clínica y su enseñanza en la formación médica.
