Abstract (English):	The mistakes of medical prescriptions represent one of the most important causes why the patients suffer risks related to the medications and constitute one of the greatest mistakes of medications notified in a very frequent way. The systems of electronic prescription are one of the most adequate tools to assure the quality of the prescription reducing the medication mistakes. The programs of therapeutic exchange contribute to facilitate the transition and continuity of pharmacotherapeutic among different social service levels and promote the rational use of the medications. These programs applied mainly to pharmacists can generate problems of availability of the medication, as well as a risk of therapeutic duplicity. The application of the therapeutic exchange program at the same moment the medical prescription is given can contribute in an important manner and increase the security of the process and facilitate the social service transition in the moment of the hospital admission. The experience in our center has shown that the application of the therapeutical exchange done by the physician needs of a simple efficient and of rapid integrated application tool in the electronic prescription which makes the process easier . Therefore the proposal of this work was centered in the objective of creating a project based in the use of the electronic prescription program as aid to the instauration of the program of therapeutic exchange. Through the assistance of the electronic data processing and pharmacy of our center it was proceeded to define the key points of this project requirements of the database, design and requirements of the computer science tool, and identification of the different phases, of the process. The use of this program of electronic prescription can be a useful tool in the reduction of mistakes derived of the application of the exchange therapeutic program, helping in the improvement of the prescription and at the same time the security of the patient. Abstract (Spanish):	Los errores de prescripción médica representan uno de los motivos más importantes por los que los pacientes sufren perjuicios relacionados con los medicamentos, y uno de los errores de medicación notificados con más frecuencia. Los sistemas de prescripción informatizados son una de las herramientas más adecuadas para asegurar la calidad de la prescripción y reducir los errores de medicación. Los programas de intercambio terapéutico contribuyen a facilitar la transición y la continuidad farmacoterapéutica entre los diferentes niveles asistenciales e impulsan el uso racional de los medicamentos. Estos programas, aplicados mayoritariamente por farmacéuticos, pueden generar problemas de disponibilidad de la medicación, así como un riesgo de duplicidad terapéutica. La aplicación del programa de intercambio terapéutico en el mismo momento en que se realiza la prescripción médica podría contribuir de manera significativa a incrementar la seguridad del proceso y facilitar la transición asistencial en el momento del ingreso hospitalario. La experiencia en nuestro centro ha demostrado que la aplicación del intercambio terapéutico por parte del médico necesita una herramienta sencilla, eficaz y de rápida aplicación, integrada en la prescripción electrónica, que facilite el proceso. Por tanto, el planteamiento de este trabajo tuvo como objetivo crear un proyecto basado en la utilización del programa de prescripción electrónica como ayuda a la implantación del programa de intercambio terapéutico. Mediante la colaboración de los servicios de informática y farmacia de nuestro centro, se procedió a definir los puntos clave del proyecto: requerimientos de la base de datos, diseño y requerimientos de la herramienta informática e identificación de las diferentes fases del proceso. La utilización de un programa de prescripción electrónica puede ser una herramienta útil en la reducción de los errores derivados de la aplicación del programa de intercambio terapéutico, contribuyendo a la mejora en la prescripción y a la seguridad del paciente.
