India faces a significant HIV burden, with female sex workers (FSWs) being highly vulnerable. This study estimated the prevalence of HIV among FSWs in India and identified risk factors. PubMed, Scopus, and Embase were searched for relevant studies published up to December 2023. Data analysis was performed using R software. Nineteen studies from different regions of India were included. The pooled HIV prevalence among FSWs was 12.6% (95% CI: 8.7; 18.1), with significant regional variations: West region reported the highest prevalence at 27.2%, followed by South (17.2%), Northeast (9.6%), and East (7.5%). Major determinants of HIV prevalence included older age, early initiation into sex work, sole dependence on sex work for income, unmarried status, inconsistent condom use, and injecting drug use. These findings show the need for region-specific interventions, including regular HIV testing, condom promotion, harm reduction for drug use, sexual health education, and integrated healthcare for HIV and sexually transmitted infections to improve health outcomes among FSWs.
