| Abstract: | Objectives: The most common neuropsychiatric complication of stroke is poststroke depression, affecting around one‐third of stroke survivors. Research varies on how poststroke depression changes over time and less is known about the course of self‐reported depression in a Swedish population. The aim was therefore to investigate how self‐reported poststroke depression changes between 3 and 12 months after stroke in a Swedish registry‐based cohort. Materials and Methods: This longitudinal study included registry‐based data from quality registers Riksstroke and Väststroke. The study included 3269 patients admitted to three stroke units at the Sahlgrenska University Hospital in Gothenburg, Sweden between November 1, 2014 and December 31, 2019. The change in self‐reported depression, measured on a 1–4 scale, was statistically analyzed using the McNemar–Bowker test and presented using descriptive statistics. Results: The study sample had a median age of 74 years, with 45% females. Three months after stroke, 57% reported depression, with a significant increase at 12 months to 63% (p < 0.001). About 62% reported no change in depression score, but 27% reported an increase in depression score, and 16% reported a decrease. Approximately 32% of patients who did not report depression at 3 months reported depression after 12 months. Conclusion: The results show that many patients who could benefit from treatment are not being identified during stroke follow‐up. Future studies could investigate self‐reported depression over a longer period to better understand how poststroke depression evolves, as well as investigate factors associated with changes in poststroke depression.
